Are you a teacher in New York? If yes, you are eligible to receive CTLE credit.
Please complete this certificate and return a copy to the AFT/Share My Lesson’s approved CTLE Sponsor along with your PD certificate. You must retain your copies for eight years. It is not necessary to send a copy of this form to the Office of Teaching Initiatives unless it is requested in the event of an audit or for use in obtaining an Initial Reissuance. A separate form must be completed for each training.

Are you a member of the Illinois Federation of Teachers (IFT)? The IFT will provide Professional Development Hours for virtual webinars members attend on the Share My Lesson platform. According to ISBE criteria for PD hours, credit can only be provided for webinars directly related to student learning. A separate form must be completed for each webinar.
Please complete this certificate and upload a copy to your MyIFT account. Make sure you retain copies of your certificate and your evidence of completion for six years. Do not submit any forms or certificates to ISBE unless requested by ISBE. Remember to log your PD hours in the Educator Licensure Information System (ELIS).

Are you a member of Texas AFT? Texas AFT is a TEA-approved professional development provider and will provide PD hours for for-credit virtual webinars that members attend on the Share My Lesson (SML) platform.
Upon completion of the Share My Lesson webinar, submit the evaluation form found at the bottom of your screen. Then please complete this PD certificate, and keep a copy of the PD certificate for your records.
