In this episode, I talk about the all-or-none nature of the action potential. Action potentials/nerve impulses either happen or they don't. If the stimulus is strong enough to cause the membrane potential to reach threshold, this will start an action potential.

The action potential starts at the axon hillock and then travels along the axon all the way down to the axon terminals. Watch this video for the details.
