Adapted from Felt's free Check Your Work posters (www.instantdisplay.co.uk) - I have shrunk them and made them into a mat to add to table boxes. I have also adapted one to fit our school policy of self evaluation - larger designed to be put back to back then laminated as one matt. Enjoy!
