I created this food diary as I couldn't find one that I liked anywhere. I didn't want to make it too complex so created it for 4 days and left space for a front cover so they could design it.I have included the self-assessment sheet I created also which was used for the follow-up lesson. I used the '2 *'s and a wish' assessment technique as they were familiar with this. This was done with their knowledge of the 'Eat Well Plate.' It was done with a P4/5 class; the P5 children created another day for a future food diary in this lesson so they were 'contributing to a healthy eating plan.'
