Family Medical History Parent-Teen Homework

Students will have a parent and/or grandparent help them do this assignment. They fill in the sheet on any disease in their family, and research at least one. They only return the top of this since this information is personal. A parent should check the box if they did the research. Tell students this information is important to their health and unfortunately, if they wait too long to ask, the older grandparents who know the answers may not be around to tell them.
